Things that we need from you to start the process | プロセスを開始するために必要なもの:

Pre-Enrollment | 事前登録

  • Pre-Enrollment is done online at the following link: PRE-ENROLL
  • Please use the worksheet to get some information before starting: WORKSHEET
  • You will need to know the name of your local school district. You can use this link to look it up: LOOKUP
  • You will also need to know your FPL (Federal Poverty Level), based on your income. You can use this site to calculate: https://fplcalculator.com/

Enrollment | 入園

  • After we receive your pre-enrollment information from Wayne RESA, we can then arrange a time for you to come visit our program, and fill out the enrollment paperwork (application form) (program guidelines and requirements, etc).
    • Our building is closed for renovations this summer (June 15~August 7), so we will have to meet you off-site during these dates.
    • We will use the seven state-required eligibility factors to determine your child’s ranking on the enrollment list:
      • EF1 : household income
      • EF2 : child has a disability or developmental delay (need IEP)
      • EF3 : expulsion from a previous child care center
      • EF4 : having a language other than English at home
      • EF5 : parent(s) without a high school degree
      • EF6 : a household member who was a victim of abuse or neglect
      • EF7 : other factors (loss of a parent or sibling, teenage parents, high-risk neighborhood, exposure to toxic substances, sibling with challenges (chronic illness, disability, behavior issues)
    • We will contact you once your child has a place in our GSRP program.

Things to bring to enrollment:

  • Birth Certificate, Passport or other proof of child birthdate (BRING)
  • Parent Identification (Driver’s License, Passport, etc…   BRING)
    • 保護者の身分証明書（運転免許証、パスポートなど）をご持参ください。
  • Income documents for enrollment intake form (BRING) | 入学願書提出用収入証明書（BRING)
    • examples are paystub, tax return, etc | 例としては給与明細、納税申告書など
    • documentation for everyone’s income in the household | 世帯全員の収入のために持参する
  • お子様の出生証明書、パスポート、または生年月日を証明する書類をご持参ください。

Things we need from you before school starts | 学校が始まる前に必要なもの:

  • Child Information Record CHILD INFORMATION RECORD (please type in English)
    • チャイルドインフォメーションカード（英語でご入力下さい）
  • Parent Handbook (signed receipt form) Parent Handbook Receipt Form
    • 入園のしおり受領書（英語でご入力下さい）
  • Health Appraisal (must be returned on or before the 3rd week of school)
    • 健康診断（学校の3週目までに返却する必要があります）（英語でご入力下さい）
